The Problem with Research

I’ve been out of the office for a few days, and was way too busy catching up on all of the countless things which don’t get done when you’re not there attending to them. So perhaps I can be forgiven for not seeing it immediately. But, at the end of the day, there it was in my Inbox, leering at me, daring me to open it up. “Selenium Does Not Benefit -- and May Harm -- Most Patients With Non-Small-Cell Lung Cancer.” What? Not again… Yet another study showing that antioxidants may do more harm than good. First it was beta-carotene. Then vitamin E. Now selenium. And that’s just the antioxidants.

To top it off, the reason I was out of the office last week was that I was attending the National Advisory Council for Complementary and Alternative Medicine, the advisory body to NIH’s NCCAM. On that council, we have talked about just this issue – why do therapies which seem to make biological and physiological sense, which have some epidemiologic data to support their use, and which naturopathic physicians (and other alternatively-minded practitioners) have been using for decades (or much longer), seem to fail in double blind, randomized clinical trials?

The reasons are multiple, complex, and often convoluted. And conventional medicine is all too willing to accept a negative study of a natural therapy, proclaim that “it doesn’t work,” and wash their hands of the entire mess, while leaving the public with the impression that alternative medicine isn’t really worth much. In reality, these therapies often fail in clinical trials because those trials are designed to answer simple, straightforward questions, and as anyone who has provided care to patients can attest, clinical medicine is anything but simple and straightforward. The typical randomized, placebo-controlled, double-blind study can only answer a limited set of very specific questions, and if the wrong question is being asked, or if it is being asked in the wrong patient population, the answer may be equivocal, confusing, or just plain wrong.

In the lung cancer study I saw today (J Clin Oncol 28:7s, 2010 (suppl; abstr CRA7004)), selenium actually appeared to show benefit in one sub-group – those who had never smoked. True, that finding did not reach statistical significance, but then neither did the finding of potential harm (P=0.15). But which one made the headline? You already know the answer.

And so I began to ponder the question, “What’s wrong with research?” A part of me becomes enraged at the reductionistic, allopathic, biomedical model, which breaks things down into components so small that all synergism, all interdependence is stripped away, and then declares those components to be ineffective. Another part argues that the wrong component was selected, or was a synthetic form (although in the lung cancer study, they used selenium yeast). But ultimately, I find myself becoming offended because I believe that these therapies work… Whoa! Believe? OK, but where is the role for evidence? I used to believe that stress caused gastric ulcers. And then along came Helicobacter pylori, and I had to change my belief to match the evidence.

For naturopathic medicine to survive and thrive in the 21st century climate of evidence-based medicine, I think at least three things must happen. First, we should be willing to judge ourselves critically and objectively, and subject our therapies (and ourselves) to scientific scrutiny. Scary and intimidating? Perhaps. Essential? Absolutely.

Second, we need to step up and recruit and train naturopathic physicians to be world-class researchers. We make great doctors – our patients tell us so every day. We have yet to prove that we can also make great researchers, although we have begun to lay the groundwork to create this success. We are playing catch-up with academic institutions that have a 50-60 year head start, but we have the advantages of being able to learn from their mistakes and of having a public who demands the type of care we provide. While the schools will bear much of this burden, it will require the concerted effort of the entire profession to create the infrastructure necessary to make this reality.

Third, we should collaborate with other professions and institutions to craft the research models necessary to adequately perform “whole systems” naturopathic research. There are examples of this type of approach already existing in the health systems research literature which can be adapted to our needs. In the end, we must create and validate the tools to dethrone the randomized controlled trial as the gold standard, and construct new ways to validate clinical approaches to health issues. Much as the homeopaths of 2+ centuries ago created the proving as a way to better understand and utilize their remedies, we must refuse to be limited by the way conventional medicine views health and disease.

We must boldly strike out to create the systems needed to transform both our research questions and the systems we use to generate the answers. Only then will we be able to use the tools of research to promote and defend naturopathic medicine, instead of feeling beaten over the head by them. Research as an advantage, instead of an impediment. Imagine that.

Sunscreen: Healthy or Toxic

It’s not an easy question. Most of us know that there is a relationship between sun exposure and skin cancer, but how many know about the problems inherent in many of the lotions that are on the market - there to supposedly limit damage to our skin?

What if the products you were buying and lathering on your face and the skin of your family contained chemicals that when exposed to sunlight actually broke down into compounds that can cause cancer? What if they contained chemicals that could disrupt normal hormonal function? But the government is there to help keep us safe, isn’t it? Unfortunately, the FDA has not made a final ruling on standards for sunscreens for the past 32 years. Proposed standards would require manufacturers to provide proof of their safety, UVA and UVB protection, and waterproof stability if those claims were made.

Research found on the website of the Environmental Working Group shows that 60% of sunscreens in the U.S. contain oxybenzone as the active ingredient to prevent burning and sun damage to skin. What the purveyors of these products do not tell you is that oxybenzone, which may be easier to apply to the skin than mineral sunscreens - like zinc oxide or titanium dioxide - is absorbed into the skin in large amounts, potentially causing allergic reactions and hormone disruption. Researchers now recommend that oxybenzone not be used on children.

Vitamin A is a wonderful antioxidant and has been shown to beneficially reduce free radical damage when taken internally or applied to the skin in night creams. But using vitamin A in sunscreen has potentially carcinogenic effects. So rather than preventing skin cancer, it actually may be contributing to it. Look for “retinol palmitate” as it is sometimes called on labels.

Much safer products are on the market now. But you have to be willing to do some research and read some labels. The biggest issue with the labels is that most of them have many ingredients, and most of these ingredients are unknown and/or unpronounceable by the general public.

But here is a good point: we know that our skin is a great delivery vehicle for medications, right? Why else would there be patches for quitting smoking, birth control, and other medications? We do absorb substances readily through our skin, and yet we assume that the skin care industry, which is largely unregulated, has our best health at heart. I hate to be the one to break it to you, but it’s not true. We live in a great capitalistic society, and they want to make money. Many of the chemicals and fragrances that are added to make them appealing are very unhealthy.

I don’t expect you to go out and get a degree in chemistry to understand all of this. Here are a few hints to help:
  • More is not better. Aim for labels with fewer ingredients – not so many that you are weary to read all of them.
  • Familiar is good! If it sounds like a strange chemical that you can’t even pronounce, you might want to think twice about it.
  • “Baby” in the name does NOT mean it is safe for kids!
  • Don’t assume that there is someone knowledgeable at your health food store who prevents the dangerous lotions from being put on the shelves.
  • Mineral sunscreens – zinc oxide or titanium dioxide – provide physical barriers to the sun yet are not absorbed into the skin.
  • Check www.ewg.org to see if your sunscreen is safe or to find one that is (as well as read more in-depth research on sunscreen ingredients).
  • Use other protective barriers against the sun, such as clothing, hats, and umbrellas. And avoid being in the noonday sun when the UV rays are potentially most damaging.
We all love this surge in warm weather and ability to spend some time outside. Spring and summer are a wonderful time for enjoying nature. Soak up your summer rays safely.

It’s Time to Bust the Myths About Chocolate

Myth: The darker the chocolate, the higher content of cacao and better for you.
Fact: Dark can be a misleading term. Chocolate in its untreated natural state is a yellowish-brown color, but “dutching” or alkalinizing, a common practice in the baking/food industry makes the chocolate a deeper, richer brown color. Acidity in chocolate is caused by the high polyphenol content and changing the pH by “dutching” imparts a milder flavor.

Myth: It is better to remove the cocoa butter from chocolate.
Fact: Most companies do not leave the cocoa butter in chocolate because they can sell it at a higher price on the secondary market. Some companies will leave a percentage of the cocoa butter in their chocolate for a creamier texture. Cocoa butter is a natural fat and has a combination of stearic acid which is cholesterol neutral, and mono-or polyunsaturated fat which can reduce cholesterol levels, so leaving a little cocoa butter in your chocolate can be a good thing.

KEY Myth: Most Chocolate has antioxidants.
Fact: An unfortunate problem in the chocolate industry is that most chocolate is processed above 110 degrees which greatly reduces the flavonol/antioxidant content. Very few chocolate companies care about this since the industry emphasis is placed on appearance and flavor. It is a rare experience to have a piece of RAW, ORGANIC, high antioxidant chocolate but it’s out there, you just have to search for it.

Myth: It is important to choose a Fair Trade chocolate.
Fact: Fair Trade means the farmer will be paid a fair wage for his cocoa beans. The downside to Fair Trade is that some farmers who produce inferior quality cacao beans are still paid the same as if they produced good quality beans. Choose a company that goes beyond Fair Trade and takes a personal interest in visiting and supporting the indigenous culture. Some companies give a bigger percentage back to the community and/or foster a nursery where farmers can work and stay on their land and not have to leave their families to work in the city for subsistence wages.

Myth: Chocolate made from Criollo variety tastes better.
Fact: Just like wines, cacao beans differ by region of origin, harvesting methods, etc. Criollo is touted as the richest, most expensive and is only 1% of the world production. Exceptionally rare criollo is harvested by dugout canoe deep in the Amazon rainforest. Criollo has less bitterness and astringency due to the lack of polyphenols. The strong and bitter tasting Forasterio variety, coming from W. Africa and S. America, comprises 80% of the worlds’ production. Trinitario comes from Ecuador and Columbia is a deeply flavorful bean with qualities of both varieties. The Arriba National variety unique only to Pacari/Ecuador was just rated #1 in the U.S. in the 70% Dark Chocolate Bar category.
